The cost components, nationally
TRT pricing in the US has four cost components, in declining order: provider supervision, pharmacy and medication, ancillary medications (like anastrozole or hCG where used), and onboarding fees. Each component varies based on the model.
What drives the price
| Higher cost | Lower cost | |
|---|---|---|
| Pharmacy | 503A compounded specialty doses | Commercial cypionate vials |
| Formulation | Pellets (quarterly), gels | Self-injection cypionate |
| Supervision | In-clinic, multi-marker monitoring | Telehealth, minimal monitoring |
| Ancillaries | Daily anastrozole, weekly hCG | Cypionate alone |
Insurance and FSA/HSA
TRT is sometimes covered by insurance when prescribed for confirmed hypogonadism with appropriate diagnostic codes. Coverage varies by plan. Many longevity-context TRT prescriptions are self-pay because the broader program is outside insurance-reimbursed care. FSA and HSA dollars are typically eligible for the medication and clinical care.
